Transitions At Home is seeking a Full-Time Physical Therapist Assistant to help patients regain independence in their own homes. The PTA will work independently with patients, following physician orders and plans of care, delivering compassionate, patient-centered care.

• Work under the supervision of the Administrator/Clinical Manager in a one-on-one setting.
• Provide high-quality compassionate care and document progress as required.

What does a Transitions at Home Physical Therapist Assistant do?

An Physical Therapist Assistant (PTA) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Administrator/Clinical Manager.

Hours: Full-Time

• 8:00am - 4:30pm
• Monday through Friday
• No Holidays

Qualifications / Job Requirements:

• Active Wisconsin Physical Therapist Assistant License
• Two (2) years’ experience preferred.
• CPR Certification
• A valid Wisconsin Driver's License and Car Insurance
