Job summary

Trinity Healthcare Talent Services is seeking a Physical Therapist in Wisconsin for a Skilled Nursing Facility and associated assisted living services. The role offers a 35-hour work week, Monday through Friday, with no weekend rotation, and an hourly salary between $39 and $45.

In this position you will evaluate patients, develop and implement treatment plans, provide hands-on therapy, and coordinate care with a multidisciplinary team.

Qualifications

• Must hold a valid Physical Therapy license in the state of [state].
• Minimum of 1 year experience as a Physical Therapist in a SNF setting preferred.
• Strong communication and interpersonal skills.
• Ability to work independently as well as part of a team.
• Excellent time management and organizational skills.
• Experience with electronic medical records preferred.

Responsibilities

• Conduct thorough evaluations of patients to determine their physical therapy needs and develop individualized treatment plans.
• Provide hands-on therapy to patients, including exercises, manual therapy, and other modalities as needed.
• Monitor and document patient progress, adjusting treatment plans as necessary.
• Communicate with other healthcare professionals, including occupational therapists and speech language pathologists, to coordinate patient care.
• Educate patients and their families on home exercise programs and techniques to improve overall function and mobility.
• Maintain accurate and timely documentation of patient progress and treatment plans.
• Collaborate with interdisciplinary team members to ensure comprehensive and effective patient care.

Facility: Skilled Nursing Facility (SNF) with short term and long term rehabilitation services, as well as a 17 bed assisted living facility (ALF)

Services: Offering occupational therapy (OT), physical therapy (PT), and speech language pathology (SLP) services to patients in both the SNF and ALF settings.
