Job summary

Trinity Healthcare Talent Services, LLC is seeking a Physical Therapist for a Skilled Nursing Facility in Wisconsin. The role offers a 35-hour workweek, Monday through Friday, with no weekend rotation.

You will conduct patient evaluations, develop individualized treatment plans, deliver hands-on therapy, and coordinate care with an interdisciplinary team to support short-term and long-term rehabilitation services.

Qualifications

• Must hold a valid Physical Therapy license in the state.
• Minimum 1 year experience as a PT in SNF setting.
• Strong communication and interpersonal skills.
• Ability to work independently as well as part of a team.
• Excellent time management and organizational skills.
• Experience with electronic medical records preferred.

Responsibilities

• Conduct thorough evaluations of patients to determine their physical therapy needs and develop individualized treatment plans.

Position: Physical Therapist

Salary Range: $39-45/hour

Schedule: 35 hours per week, Monday-Friday, no weekend rotation

Facility: Skilled Nursing Facility (SNF) with short term and long term rehabilitation services, as well as a 17 bed assisted living facility (ALF)

Services: Offering occupational therapy (OT), physical therapy (PT), and speech language pathology (SLP) services to patients in both the SNF and ALF settings.
