Trinity Healthcare Talent Services, LLC is seeking a licensed Physical Therapist for a full-time position in Santa Fe, NM, at a skilled nursing facility serving short-term and long-term patients.

You will evaluate patients, develop treatment plans, use exercise, manual therapy, and education, and document progress while collaborating with a care team.

This role requires at least 2 years of PT experience, current state license, CPR certification, and strong communication skills.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Current state license to practice as a Physical Therapist.
• CPR certification required.
• Minimum of 2 years of experience in skilled nursing or similar setting.

Responsabilités

• Evaluate patients' medical history and perform physical examinations to determine PT needs.
• Develop and implement personalized treatment plans to improve mobility, reduce pain, and increase strength.
• Utilize techniques such as exercise, massage, and manual therapy to reach treatment goals.
• Monitor and document progress, adjust plans as needed, and communicate with other healthcare professionals for coordinated care.
• Educate patients and families about condition, plan, and home exercises to promote independence.
• Collaborate with therapists and healthcare team for comprehensive care.
• Stay current with PT developments and engage in continuing education.

Position: Physical Therapist

Salary Range: $44.80 - $49.23 per hour

Hours: Full-time, 35 hours per week, Monday - Friday (No weekend rotation)

Location: Skilled nursing facility with mix of short term and long term care patients

Key Responsibilities:

• Evaluate patients' medical history and perform physical examinations to determine their physical therapy needs
• Develop and implement personalized treatment plans to help patients improve their mobility, reduce pain, and increase strength and endurance
• Utilize a variety of techniques, such as exercise, massage, and manual therapy, to help patients achieve their treatment goals
• Monitor and document patients' progress, adjust treatment plans as needed, and communicate with other healthcare professionals to ensure coordinated care
• Educate patients and their families about their condition, treatment plan, and home exercises to promote independence and long-term wellness
• Collaborate with other therapists and healthcare team members to provide comprehensive care for patients
• Stay up-to-date with the latest developments in physical therapy and participate in continuing education to enhance skills and knowledge

Qualifications:

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
• Current state license to practice as a Physical Therapist
• Minimum of 2 years of experience working as a Physical Therapist, preferably in a skilled nursing facility or similar setting
• Strong knowledge of physical therapy techniques and best practices
• Excellent communication and interpersonal skills to work effectively with patients, families, and other healthcare professionals
• Ability to work independently and as part of a team to provide high-quality care to patients
• CPR certification is required
