Job summary

Dove Healthcare is seeking a licensed Physical Therapist for casual openings at its St. Croix Falls location.

You will plan and direct physical therapy services in a skilled nursing facility, participate in discharge planning, and contribute to patient assessments and care plans while maintaining detailed treatment records. Qualifications

• Current Wisconsin license as a physical therapist.
• Eligibility for licensure as a Physical Therapist in Wisconsin.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct physical therapy services in a skilled nursing facility.
• Participate in discharge planning and development of patient care plans.
• Maintain treatment records, patient files, and progress notes.
• Contribute to patient assessments and quarterly plans.
• Collaborate with and supervise PTAs.

As a physical therapist for Dove Healthcare, you will

• plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
• participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
• maintain treatment records, patient files, and progress notes as required
• participate in the development and implementation of patient assessments and care plans, including quarterly reviews
• collaborate and supervise PTAs

Required Qualifications

• doctorate degree (or bachelor's / master's degree for those grandfathered in) in physical therapy from an accredited college or university
• current license as a physical therapist in the State of Wisconsin
