Job summary

Dove Healthcare in St. Croix Falls, WI, is seeking a licensed physical therapist to lead therapy services in our skilled nursing facility.

You will participate in discharge planning, develop patient care plans, assess patients, and maintain treatment records. New graduates are encouraged to apply, with competitive benefits and shift differentials.

Qualifications

• Doctorate in Physical Therapy (or bachelor's/master's for those grandfathered).
• Current Wisconsin license to practice physical therapy.

Responsibilities

• Plan, develop, organize, implement, and direct physical therapy and programs at a skilled nursing facility.
• Participate in discharge planning and patient care plan development and assessments.
• Maintain treatment records, files, and progress notes.
• Contribute to patient assessments and care plans, including quarterly reviews.
• Collaborate with and supervise PTAs.

As a physical therapist for Dove Healthcare, you will

• plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
• participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
• maintain treatment records, patient files, and progress notes as required
• participate in the development and implementation of patient assessments and care plans, including quarterly reviews
• collaborate and supervise PTAs

Required Qualifications:

• doctorate degree (or bachelor's / master's degree for those grandfathered in) in physical therapy from an accredited college or university
• current license as a physical therapist in the State of Wisconsin
