Job summary

Dove Healthcare is seeking a licensed Physical Therapist to join its St. Croix Falls team. The PT will plan, develop, and direct therapy programs in a skilled nursing facility, participate in discharge planning and family/patient care planning, and maintain accurate documentation.

Qualifications

• Doctorate, orBachelor’s/Master’s for grandfathered in, in physical therapy from an accredited college or university.
• Current license as a physical therapist in the State of Wisconsin.

• Plan, develop, organize, implement, evaluate, and direct physical therapy and its programs in a skilled nursing facility.
• Participate in discharge planning and development/implementation of patient care plans.
• Maintain treatment records, patient files, and progress notes.

As a physical therapist for Dove Healthcare, you will

• plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
• participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
• maintain treatment records, patient files, and progress notes as required
• participate in the development and implementation of patient assessments and care plans, including quarterly reviews
• collaborate and supervise PTAs

Required Qualifications

• doctorate degree (or bachelor's / master's degree for those grandfathered in) in physical therapy from an accredited college or university
• current license as a physical therapist in the State of Wisconsin
