Job summary

Trinity Healthcare Talent Services, LLC is seeking a Physical Therapist for a skilled nursing facility, focusing on mobility and independence. You will partner with physicians, nurses, and therapists to tailor treatment plans for residents.

We offer a Monday–Friday schedule (35 hours/week) with no weekend rotation in Mount Olive, North Carolina. Active PT license and 1–2 years SNF experience are preferred for compassionate, collaborative care.

Qualifications

• Bachelor's/Master's/Doctorate in Physical Therapy from an accredited program.
• Active state Physical Therapist license.
• 1–2 years of experience in a skilled nursing facility or similar setting.
• Strong knowledge of therapeutic techniques and modalities.

Responsibilities

• Conduct initial evaluations and assessments to determine the patient's physical abilities and limitations.
• Develop and implement personalized treatment plans based on the patient's needs and goals.
• Utilize various therapeutic techniques to improve strength, range of motion, balance, and coordination.
• Instruct and supervise patients in therapeutic exercises and activities.
• Monitor and document patient progress and adjust treatment plans as needed.
• Educate patients and their families on home exercise programs and injury prevention techniques.
• Collaborate with other members of the healthcare team to ensure the best possible care.

Job Description:
