Job summary

Trinity Healthcare Talent Services, LLC is seeking a skilled Physical Therapist to join our team at a 164-bed Skilled Nursing Facility. The role focuses on developing individualized treatment plans to help patients maximize their physical potential and improve quality of life.

This full-time position offers a Monday–Friday schedule (40 hours) with no weekend rotation and requires a state license, a degree in Physical Therapy, and CPR certification.

Qualifications

• Bachelor's or Master's degree in Physical Therapy.
• State license to practice as a Physical Therapist.

• Experience in a Skilled Nursing Facility or similar setting preferred.

Responsibilities

• Conduct thorough evaluations of patients' physical abilities and limitations
• Develop personalized treatment plans based on evaluation results and patients' goals
• Utilize a variety of techniques and exercises to help patients improve their mobility, strength, and range of motion
• Monitor and document patients' progress throughout their treatment
• Collaborate with other healthcare professionals to ensure coordinated and comprehensive care for patients
• Educate patients and their families on exercises and techniques to continue at home
• Stay up-to-date with advancements in the field of physical therapy and incorporate new techniques into treatment plans
