Job summary

Palms Home Care is seeking a Home Health Physical Therapist to provide in-home therapy to patients, helping them regain mobility and independence after illness or surgery.

You will perform evaluations, develop individualized plans, guide exercises, document progress, and collaborate with care teams while ensuring compliance with home health guidelines.

Qualifications

• Licensed Physical Therapist in the state of Florida.
• 1–2 years of home health experience preferred.
• Eligible to provide home health PT services.

Responsibilities

• Provide therapeutic exercises, manual therapy, and mobility training.
• Assess patient progress and adjust care plans accordingly.

Our company is expanding and we are seeking a skilled Home Health Physical Therapist (PT) to join our team. • Experience with EMR systems such as Kinnser
