Job summary

Dove Healthcare is seeking a Physical Therapist to join our Fennimore location. You will plan and direct physical therapy programs within a skilled nursing facility, ensuring high-quality care and adherence to regulations.

Responsibilities include discharge planning, patient assessments, treatment documentation, and collaboration with PTAs. Wisconsin licensure and a PT degree are required; new graduates are encouraged to apply.

Qualifications

• Doctorate degree in physical therapy, or bachelor’s/master’s as grandfathered in.
• Current Wisconsin physical therapist license.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct physical therapy and related programs in a skilled nursing facility.
• Participate in discharge planning, development and implementation of patient care plans and assessments.
• Maintain treatment records, patient files, and progress notes.
• Contribute to patient assessments and care plans, including quarterly reviews.
• Collaborate with and supervise PTAs.

As a physical therapist for Dove Healthcare, you will

• plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
• participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
• maintain treatment records, patient files, and progress notes as required
• participate in the development and implementation of patient assessments and care plans, including quarterly reviews
• collaborate and supervise PTAs

Required Qualifications

• doctorate degree (or bachelor's / master's degree for those grandfathered in) in physical therapy from an accredited college or university
• current license as a physical therapist in the State of Wisconsin
