Job summary

Dove Healthcare is hiring a Physical Therapist for our St. Croix Falls location in Wisconsin. You will plan, implement, and evaluate physical therapy services within a skilled nursing facility, ensuring high-quality patient care and adherence to regulations.

Qualifications

• DPT (or Bachelor\'/Master\'s in Physical Therapy for those grandfathered) is required.
• Current license as a Physical Therapist in Wisconsin is required.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct physical therapy programs in a skilled nursing facility.
• Participate in discharge planning and development of patient care plans.
• Maintain treatment records, patient files, and progress notes.
• Contribute to patient assessments and quarterly care plan reviews.
• Collaborate with and supervise PTAs.

As a physical therapist for Dove Healthcare, you will

• plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
• participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
• maintain treatment records, patient files, and progress notes as required
• participate in the development and implementation of patient assessments and care plans, including quarterly reviews
• collaborate and supervise PTAs

Required Qualifications:

• doctorate degree (or bachelor's / master's degree for those grandfathered in) in physical therapy from an accredited college or university
• current license as a physical therapist in the State of Wisconsin
