Job summary

BrightStar Care of Concord, Lexington and Woburn is actively seeking a licensed Physical Therapist to perform in-home evaluations and develop personalized care plans. You will deliver skilled therapy services focused on improving strength, balance, gait, and functional mobility in clients in their own homes.

Education and independence are emphasized, with collaboration across nurses, OTs, HHAs, and case managers. Flexible hours, competitive pay, and opportunities for growth are offered.

Qualifications

• Active and unrestricted Massachusetts Physical Therapist license.
• Graduate of an accredited PT program (DPT or MPT).
• Minimum 1 year PT experience preferred (new grads welcome with supervision).
• Reliable vehicle and valid driver’s license.
• CPR certification.
• Ability to work independently and as part of a care team.

Responsibilities

• Perform in-home physical therapy evaluations and develop personalized care plans.
• Deliver skilled therapy services focused on improving strength, balance, gait, and functional mobility.
• Educate patients and family members on home exercise programs and safe mobility practices.
• Collaborate with nurses, OTs, HHAs, and case managers.
• Document patient progress and visit notes using a mobile EMR system.
• Ensure patient safety and quality outcomes aligned with BrightStar’s standards.
