Job summary

BrightStar Care of Concord, Lexington and Woburn is seeking a licensed Physical Therapist to provide in-home and facility-based services with a focus on mobility, strength, and safe home exercise programs. You’ll work within a supportive team and utilize mobile EMR for documentation.

Qualifications

• Active and unrestricted Massachusetts Physical Therapist license.
• Graduate of an accredited PT program (DPT or MPT).
• Minimum 1 year of PT experience preferred (new grads welcome with supervision).
• Reliable vehicle and valid driver’s license.
• CPR certification.
• Ability to work independently and as part of a care team.

Responsibilities

• Perform in-home physical therapy evaluations and develop personalized care plans.
• Deliver skilled therapy services focused on improving strength, balance, gait, and functional mobility.
• Educate patients and family members on home exercise programs and safe mobility practices.
• Collaborate with nurses, OTs, HHAs, and case managers.
• Document patient progress and visit notes using the mobile-friendly EMR system.
• Ensure patient safety and quality outcomes aligned with BrightStar’s standards and compliance protocols.
