Job summary

BrightStar Care of Concord, Lexington and Woburn is seeking a licensed Physical Therapist to provide in-home and facility-based services. You will evaluate, design, and implement personalized therapy plans to restore mobility and independence.

You will educate patients and families on safe exercise programs, collaborate with a multidisciplinary BrightStar team, and document progress via a mobile EMR system. A flexible schedule and growth opportunities are offered.

Qualifications

• Active and unrestricted Massachusetts Physical Therapist license.
• Graduate of an accredited PT program (DPT or MPT).
• Minimum 1 year of PT experience preferred (new grads welcome with supervision).
• Reliable vehicle and valid driver’s license.
• CPR certification.
• Ability to work independently and as part of a care team.

Responsibilities

• Perform in-home PT evaluations and develop personalized care plans.
• Deliver skilled therapy to improve strength, balance, gait, and mobility.
• Educate patients and families on home exercise and safe mobility.
• Collaborate with nurses, OTs, HHAs, and case managers.
• Document progress and visit notes using a mobile EMR system.
• Uphold safety and quality standards and compliance.
