Job summary

Embassy of Heartside in State College, PA is seeking a compassionate full-time Physical Therapist to join our in-house rehabilitation team. New graduates are welcome to apply; you will help residents improve mobility, strength, balance, and independence by developing personalized treatment plans.

Qualifications

Responsibilities

• Evaluate residents’ mobility, strength, balance, and functional needs.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled therapy to reduce pain, restore function, and promote healing.
• Monitor progress and adjust treatment plans as needed; educate residents and families.
• Collaborate with interdisciplinary care team and document accurately.
• Ensure services meet professional, legal, and regulatory standards.

Key Responsibilities

• Evaluate residents’ mobility, strength, balance, and functional needs
