Job summary

Innovate Rehab and Wellness is seeking a Licensed Physical Therapist to join a full-time team at our skilled nursing facility in St. Louis, MO.

You will evaluate patients, lead individualized treatment plans, and provide hands-on therapy to restore movement and quality of life. You will work closely with an interdisciplinary team to optimize recovery, with a schedule Monday–Friday and a competitive hourly pay based on experience.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• CPR certification preferred.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
