INNOVATE REHAB AND WELLNESS in Winnfield, LA seeks a dedicated Licensed Physical Therapist (PT) to provide patient-centered care in a skilled nursing facility. This full-time role involves evaluating patients, creating individualized treatment plans, and collaborating with an interdisciplinary team for optimal recovery.

Position Summary

As a Physical Therapist, you will:

• Evaluate patients and identify movement and functional limitations
• Create and lead individualized treatment plans
• Help restore movement, function, and quality of life
• Work closely with an interdisciplinary team for optimal recovery

Key Responsibilities

• Perform comprehensive PT evaluations
• Develop personalized, evidence-based treatment plans
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training
• Educate patients and caregivers on home programs and injury prevention
• Track and document progress; adjust plans as needed
• Collaborate with physicians, nurses, OTs, and other clinicians
• Stay current with PT best practices through continuing education
• Ensure compliance with all regulatory and ethical standards
