INNOVATE REHAB AND WELLNESS in St. Louis, MO is seeking a Licensed Physical Therapist for a full-time role in a skilled nursing facility. The position offers a Monday–Friday schedule with a competitive hourly rate based on experience and collaborative care with an interdisciplinary team.

Responsibilities include comprehensive evaluations, individualized treatment plans, hands-on therapy, patient education, and progress documentation, with opportunities for professional growth and development.

Qualifications

• Doctorate or Master's degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.
• Strong communication, problem-solving, and patient-care skills.
• CPR certification preferred.

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands‑on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with all regulatory and ethical standards.
