Job summary

Innovate Rehab and Wellness is seeking a Licensed Physical Therapy Assistant (PTA) to join our skilled nursing facility team. You will work under a licensed Physical Therapist to help patients improve mobility, regain strength, and increase function through individualized treatment plans.

Responsibilities include leading exercises, mobility training, and rehab activities, educating patients on safe techniques and devices, and maintaining accurate records.

Qualifications

• Valid state PTA license required.
• Associate degree from an accredited PTA program.
• Strong communication and interpersonal skills.
• CPR certification preferred.

Responsibilities

• Implement patient-specific treatment plans created by the PT.
• Lead patients through exercises, mobility training, and rehab activities.
• Educate patients on safe techniques and assistive devices.
• Monitor and document patient progress; report changes to the PT.
• Maintain accurate therapy records and collaborate with the care team.
