Job summary

Innovate Rehab and Wellness is seeking a Licensed Physical Therapist (PT) for a full-time position in a skilled nursing facility. You will evaluate patients, develop individualized treatment plans, and lead one-on-one therapy sessions Monday–Friday, with a competitive hourly rate based on experience.

You will work closely with an interdisciplinary team to restore movement, function, and quality of life, while ensuring compliance with regulatory standards and ongoing professional development.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• CPR certification preferred; strong communication and patient-care skills.

Responsibilities

• Perform comprehensive PT evaluations and identify movement limitations.
• Develop personalized, evidence-based treatment plans and lead sessions.
• Provide hands-on therapy, manual techniques, and therapeutic exercises.
• Educate patients and caregivers on home programs and injury prevention.
• Track progress and adjust plans; document changes.
• Collaborate with physicians, nurses, OTs, and other clinicians.
