Concept Therapy PC and Concept Therapy Home Health in South Bend area are seeking a compassionate Physical Therapist to join outpatient and home health teams. We are expanding and looking for a compassionate, motivated Physical Therapist to join our team. This role includes opportunities in both outpatient therapy and skilled home health, allowing you to practice across settings and support patients throughout their full continuum of care.

Job Summary

As a Physical Therapist with Concept Therapy, you will work with patients to evaluate their mobility and functional needs, and develop customized therapy plans that help them reach meaningful goals. You will collaborate closely with physicians, nurses, therapists, and caregivers to ensure coordinated follow‑up care and provide education that empowers patients to thrive.

The ideal candidate is compassionate, organized, communicative, and comfortable managing a varied caseload across outpatient and home health environments.
