Concept Rehab is seeking a compassionate Physical Therapist to join our skilled nursing and post-acute care team in Cincinnati, OH. The role emphasizes comprehensive evaluations, evidence-based interventions, and collaboration with physicians, nurses, and families to restore independence.

• Perform comprehensive physical therapy evaluations and establish individualized treatment plans.
• Deliver evidence-based interventions focused on mobility, balance, strength, pain management, and functional independence.
• Monitor patient progress and adjust treatment strategies to maximize outcomes.

• Provide supervision and direction to Physical Therapy Assistants in accordance with state regulations.

Requirements

What You'll Do

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Ohio Physical Therapist license in good standing
• Skilled nursing, post‑acute, or geriatric rehabilitation experience preferred
• Excellent communication and clinical reasoning skills
• Ability to build strong relationships with patients, families, and healthcare professionals
