Job summary

Concept Rehab is seeking a compassionate part-time Physical Therapist to join our rehabilitation team in a skilled nursing and post-acute care setting in Ohio. You will conduct comprehensive evaluations, develop individualized plans, and deliver evidence-based interventions to restore mobility, balance, strength, and functional independence.

Remote work is not offered. The role emphasizes collaboration with physicians and nursing staff, patient education, and participation in quality improvement

Qualifications

• Excellent communication and clinical reasoning skills.
• Ability to collaborate with physicians, nursing staff, families, and interdisciplinary teams.
• Strong patient education and engagement abilities.
• New graduates are encouraged to apply.
• Current Ohio Physical Therapist license in good standing.
• Experience in skilled nursing or post-acute rehabilitation is preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations and establish individualized treatment plans.
• Deliver evidence-based interventions focused on mobility, balance, strength, pain management, and functional independence.
• Monitor patient progress and adjust treatment strategies to maximize outcomes.
• Collaborate with physicians, nursing staff, families, and interdisciplinary care teams.
• Provide supervision and direction to Physical Therapy Assistants in accordance with state regulations.
• Complete timely, accurate, and compliant clinical documentation.
• Educate patients and caregivers on safe mobility techniques and discharge recommendations.
• Participate in care conferences, team meetings, and quality improvement initiatives.
• Promote a positive patient experience through compassionate and patient-centered care.

• Graduate of an accredited Physical Therapy program
• Current Ohio Physical Therapist license in good standing
• Skilled nursing, post-acute, or geriatric rehabilitation experience preferred
• Excellent communication and clinical reasoning skills
• Ability to build strong relationships with patients, families, and healthcare professionals
