Concept Therapy PC and Concept Therapy Home Health are seeking an Occupational Therapist to join our team. You will work across outpatient clinics and in-home skilled care, serving patients in South Bend / Mishawaka / Elkhart, IN.

You will evaluate functional needs, create customized therapy plans, and collaborate with physicians, nurses, and caregivers to ensure coordinated follow‑up care and support patient empowerment.

Qualifications

• Active Occupational Therapy license in Indiana.
• Strong communication and interpersonal skills.
• Excellent organizational abilities and attention to detail.
• Ability to manage multiple cases while maintaining a compassionate approach.

Responsibilities

• Meet with patients from diverse backgrounds and discuss therapy needs with empathy and professionalism.
• Conduct thorough evaluations to establish functional baselines.
• Develop individualized therapy plans that support mobility, independence, and daily living skills.
• Provide outpatient treatment in clinic settings and skilled therapy services in the home.
• Collaborate with interdisciplinary care teams to ensure follow‑up and continuity of care.
• Educate patients and families on exercises, safety, adaptive strategies, and lifestyle modifications.
• Maintain accurate, detailed documentation of therapy sessions and patient progress.
