Job summary

Palms-Home-Care in Sebring, FL is seeking a skilled Home Health Physical Therapist to provide personalized therapy in patients' homes, helping them regain mobility and independence after illness or surgery.

You will perform initial evaluations, develop treatment plans, guide therapeutic exercises, monitor progress, educate patients and caregivers, and ensure accurate documentation while coordinating with the care team. A Florida license and CPR certification are required.

Qualifications

• Current Physical Therapist license is required.
• Graduate of accredited PT program.
• 1–2 years of experience in Home Health preferred.
• Strong communication skills.
• Ability to work independently.
• Possesses and maintains CPR certification.
• Valid driver’s license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.
