Job summary

Palms Home Care in Florida is seeking a skilled Home Health Physical Therapist (PT) to join our team. You will provide individualized physical therapy services to patients in their homes, helping them improve mobility and regain independence after illness, injury, or surgery.

The role involves initial evaluations, developing treatment plans, therapeutic exercises, manual therapy, and mobility training, with documentation and collaboration with the care team to ensure Medicare guidelines are

Qualifications

• Current Physical Therapist license.
• Graduate of accredited PT program.
• 1–2 years of experience preferred in Home Health.
• Strong communication skills.
• Ability to work independently.
• Possesses and maintains current CPR certification.
• Valid driver’s license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.
