Job summary

Ascension St John Sapulpa is seeking a licensed Physical Therapist to evaluate and treat patients in a PRN weekend/weekday role within the Physical Therapy department. You will perform initial and ongoing assessments, establish and revise plans of care with patient/family involvement, and supervise assistants and students while delivering standard physical therapy interventions.

Qualifications

• PT license in Oklahoma required.
• BLS Provider certification within 30 days of hire.
• PT credentialed by the Oklahoma Board of Medical Licensure and Supervision.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Physical Therapy (PT) credentialed from the Oklahoma Board of Medical Licensure and Supervision prior to hire date or job transfer date required. Physical Therapist temporary letter will be accepted until license is issued.

Education

• Bachelor's degree required. Master's OR Doctorate degree preferred.
