Santa Rosa Post Acute is seeking a compassionate Physical Therapist to join our rehabilitation team in Santa Rosa, CA. You will assess residents, design individualized plans, and provide evidence-based care to restore mobility and independence.

Join a dedicated interdisciplinary team in a supportive post-acute care setting. California PT licensure is required, with SNF/post-acute experience preferred. Excellent communication and teamwork are essential.

Qualifications

• Current California Physical Therapist license required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively within an interdisciplinary healthcare team.

Responsibilities

Santa Rosa Post Acute | Santa Rosa, CA

Pay: $75.00 - $85.00 per hour

SIGN-ON BONUS: $2,000

Full-time new hires are eligible for a $2,000 sign-on bonus , paid in two installments:

• $1,000 (50%) after completion of 90 days of continuous employment
• $1,000 (50%) after completion of 180 days of continuous employment

Bonus payments are contingent upon remaining actively employed and in good standing at the time each payment is due.

Physical Therapist Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide evidence-based physical therapy services to improve mobility, strength, balance, and functional independence
• Assess patient needs and establish measurable treatment goals
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with physicians, nursing staff, therapy team members, and families
• Educate patients and caregivers regarding treatment plans, exercises, and safety recommendations
• Participate in interdisciplinary care planning meetings and patient conferences
• Supervise Physical Therapist Assistants (PTAs) as appropriate
• Recommend adaptive equipment and mobility aids when necessary
• Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines

Qualifications

• Current California Physical Therapist (PT) License required
• Graduate of an accredited Physical Therapy program

• Excellent communication and interpersonal abilities
• Ability to work effectively within an interdisciplinary healthcare team
• Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, or Long-Term Care experience preferred
