Job summary

Sonoma Post Acute in Sonoma, CA is seeking a licensed Physical Therapist to join our rehabilitation team in a post-acute setting. The role offers full-time, part-time, or per diem hours with competitive pay.

You will evaluate residents, develop individualized plans, and work with an interdisciplinary team to improve mobility and independence. A California license is required; SNF/post-acute experience is preferred, and strong communication and organizational skills are essential.

Qualifications

• Current and unrestricted California Physical Therapist License required.
• Graduate of an accredited Physical Therapy program.

• Strong clinical assessment and treatment skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services in a SNF/post-acute setting.
• Improve mobility, strength, balance, endurance, and independence.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing and other therapists.
• Educate patients and families on goals and home exercise programs.
• Complete accurate clinical documentation per state and federal rules.
• Contribute to positive outcomes and quality-of-care initiatives.

Responsibilities

• Improve patient mobility, strength, balance, endurance, and functional independence
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals
• Educate patients and families regarding treatment goals, recovery expectations, and home exercise programs
• Complete accurate and timely clinical documentation in compliance with state and federal regulations
• Contribute to positive patient outcomes and quality-of-care initiatives

Qualifications

• Current and unrestricted California Physical Therapist License required
• Graduate of an accredited Physical Therapy program
• Skilled Nursing Facility (SNF), rehabilitation, long-term care, or post-acute experience preferred
• Strong clinical assessment and treatment skills
• Excellent communication, organization, and time-management abilities
• Ability to work independently while contributing to a team-focused environment
• Commitment to delivering patient-centered care
