Job summary

Santa Rosa Post Acute in Santa Rosa, CA is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team. The role offers hourly pay and both full-time and part-time options, with integration into a collaborative, interdisciplinary setting.

You will evaluate residents, develop individualized treatment plans, and help patients regain independence through ADLs, mobility, and cognitive training. Our facility emphasizes patient outcomes and employee satisfaction.

Qualifications

• Current California Occupational Therapist license in good standing.
• Graduate of an accredited Occupational Therapy program.
• Experience in skilled nursing, post-acute, rehabilitation, or geriatric settings preferred.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Provide skilled OT interventions per physician orders and care plans.
• Assist patients with ADLs, mobility, cognition, and independence.
• Collaborate with PTs, SLPs, nursing, and families to coordinate care.
• Complete timely documentation in compliance with regulations.
• Participate in interdisciplinary meetings and discharge planning.
• Educate patients and families on therapeutic techniques and home programs.
• Maintain compliance with professional standards.

$72-$77/hr. | Full-Time or Part-Time

Sign-On Bonus

• Full-time new hires are eligible for a $2,000 sign-on bonus, paid in two installments:
• $1,000 (50%) after completion of 90 days of continuous employment
• $1,000 (50%) after completion of 180 days of continuous employment
• Bonus payments are contingent upon remaining actively employed and in good standing at the time each payment is due.

About Santa Rosa Post Acute

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans
• Provide skilled occupational therapy interventions in accordance with physician orders and established care plans
• Help patients improve performance in activities of daily living (ADLs), self-care tasks, mobility, cognition, and functional independence
• Collaborate with Physical Therapists, Speech Therapists, nursing staff, physicians, and families to coordinate care
• Complete timely and accurate documentation in compliance with regulatory and facility requirements
• Participate in interdisciplinary team meetings, care conferences, and discharge planning
• Educate patients, families, and caregivers regarding therapeutic techniques and home programs
• Maintain compliance with all federal, state, and professional standards of practice

Qualifications

• Current California Occupational Therapist (OT) license in good standing
• Graduate of an accredited Occupational Therapy program
• Skilled nursing, post-acute, rehabilitation, or geriatric experience preferred
• New graduates with strong clinical rotations are encouraged to apply
• Experience with Casamba, Net Health, PointClickCare, or similar EMR systems is a plus
• Strong communication, organizational, and teamwork skills
• Passion for helping patients achieve their highest level of function and independence
