Parkview Post Acute in Santa Rosa, CA is seeking a dedicated Physical Therapist to join our in-house rehab program full time. The role offers a stable team, patient-centered care, and a competitive pay rate of $60–$75 per hour, with weekend premium for additional availability.

PHYSICAL THERAPIST – FULL TIME

PARKVIEW POST ACUTE – SANTA ROSA, CA

Join our IN-HOUSE Therapy Team in Santa Rosa as a Dedicated PT!

Parkview Post Acute in Santa Rosa, CA, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

FULL TIME: Flexible schedule with work-life balance

Pay: $60- $75/hour based on experience

Higher hourly rate for weekend availability

Key Responsibilities:

• Effectively screens/evaluates patients with physical functioning disorders and/or wound care and develops appropriate plan of care following all regulatory and clinical practice standards.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders and/or wound care per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including but not limited to manual techniques; ambulation; therapeutic exercises; modalities; use of supportive and assistive devices; wound care; etc.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements

Licensed Physical Therapist
