Flagship Therapy is hiring a Physical Therapist Assistant for our Summerfield Post Acute and Rehab facility in Santa Rosa, CA. This full-time role focuses on in-house rehab programs for geriatric patients, working under the supervision of a licensed Physical Therapist.

We seek dedicated team members who can apply evidence-based interventions, perform rehabilitative procedures, and collaborate closely with pharmacy and care teams to optimize patient outcomes.

Qualifications

• Must hold Physical Therapist Assistant license.
• New grads welcome.

Responsibilities

• Apply evidence-based practices to support clinical interventions.
• Provide rehabilitative treatment to patients under Physical Therapist supervision.
• Administer rehabilitation procedures including manual techniques, ambulation, exercises, modalities, and device use under supervision.
• Collaborate with Supervising Physical Therapist to assess and adjust treatments per regulatory and clinical requirements.
• Offer consultation and counseling to patients, families, caregivers, and other providers regarding physical disorders.

SUMMERFIELD POST ACUTE AND REHAB - SANTA ROSA, CA

Our expanding network of in-house therapy Ensign-affiliate operated healthcare facilities is growing, and we are scouting for excellent therapists to join us at Summerfield Post Acute and Rehab in Santa Rosa, CA. We offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking Full Time Physical Therapist Assistants for our in-house rehab program!

Key Responsibilities

• Apply evidence-based practices to support clinical interventions.
• Provide rehabilitative treatment to patients with physical functioning disorders under Physical Therapist supervision.
• Administer various rehabilitation procedures, including manual techniques, ambulation, exercises, modalities, and supportive device use, under Physical Therapist supervision.
• Collaborate with the supervising Physical Therapist to assess and adjust treatments according to regulatory and clinical practice requirements.
• Offer consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.

Qualifications

• Physical Therapist Assistant license is required
