Job summary

LiveWell Homecare Agency, LLC is seeking a licensed Physical Therapist to deliver medically prescribed therapy aimed at reducing pain, restoring function, and preventing disability after illness or injury. You will conduct evaluations, design individualized therapy programs, guide exercises, supervise staff when needed, and educate patients on mobility aids and self-management.

A DPT is mandatory; BLS certification required; pelvic health certification preferred and should be obtained within 90

Qualifications

• Must hold a current Physical Therapist license.
• BLS certification required; pelvic health certification preferred.
• Post-2015, a Doctorate in Physical Therapy (DPT) is mandatory.
• Experience in therapy and patient care preferred.

Job description

Role Overview

The Physical Therapist provides medically prescribed therapy to alleviate pain, restore function, and prevent disability after illness or injury.

Key Responsibilities

Conducts evaluations, develops individualized therapy programs using exercise, manual therapy, heat, water, light, and electrical modalities. Guides patients through active/passive exercises, muscle reeducation, gait and functional training, and instructs on the use of mobility aids and orthotic devices. Utilizes diagnostic tests for muscles, nerves, and joints, maintains patient progress records, and collaborates with the rehab team. May supervise staff and participate in training students and staff in therapy techniques.

Qualifications & Skills

• Must hold a license as a Physical Therapist.
• Required certifications include Basic Life Support (BLS), with preferred certifications in Pelvic Health.
• Post-2015, a Doctorate in Physical Therapy (DPT) is mandatory.
• Experience requirements vary; relevant experience in therapy and patient care is preferred.

Additional Requirements

Ability to perform initial and re-evaluations, utilize therapy equipment, and provide patient education. Certification in pelvic health is preferred and must be obtained within 90 days if not already held.
