Job summary

LiveWell Homecare Agency, LLC is seeking a Licensed Physical Therapist to provide medically prescribed therapy, relieve pain, restore function, and prevent disability after disease, injury, or loss of a body part.

You will develop personalized treatment programs, use modalities such as exercise, massage, manual therapy, heat, water, light, and electricity, and supervise staff or train students in therapy techniques as needed.

Qualifications

• Doctorate of Physical Therapy (post-2015) required.
• Licensure as a Physical Therapist.
• BLS certification; Pelvic Health certification preferred.
• Experience required within specified years and certification timelines (within 90 days if not already held).
• Proficiency in patient assessment, treatment planning, equipment operation, and patient education.

Responsibilities

• Conduct diagnostic tests and evaluate patients.
• Guide patients through active and passive exercises; re-educate muscles.
• Train gait and functional movements with equipment as prescribed.
• Perform initial and re-evaluations and document progress.
• Possibly supervise staff or train students/workers in therapy techniques.
