Job summary

Angels Care Home Health is seeking a Physical Therapist to plan, implement, and evaluate patient care to restore or maintain well-being. You will design therapy programs based on referrals and evaluations while coordinating with the interdisciplinary team.

The ideal candidate holds a current Texas PT license, has two years of experience, CPR and Hepatitis profile, and knowledge of durable medical equipment.

Qualifications

• Current state license as a Physical Therapist.
• Two years of experience as a Physical Therapist.
• CPR certification and Hepatitis profile are required.
• Knowledge of durable medical equipment.

Responsibilities

• Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
• Plans, organizes, and conducts physical therapy treatment programs based on the medical referral and their evaluation.
• Collaborates with interdisciplinary team.
