Job summary

Angels Care Home Health seeks a licensed Physical Therapist to assess and treat patients in a home health setting. You will plan and implement individualized PT programs, coordinating closely with an interdisciplinary team to restore or maintain patient well-being.

Ideal candidates have an active PT license, 2+ years of PT experience, CPR and Hepatitis profile, and knowledge of durable medical equipment. Flexibility to cover patient homes and collaborate across care teams is preferred.

Qualifications

• Current state license as a Physical Therapist.
• Two years of experience as a Physical Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Plans, implements and evaluates patient care plans to restore or maintain patient well being.
• Plans, organizes and conducts physical therapy treatment program based on the medical referral and their evaluation.
• Collaborates with interdisciplinary team.
