Job summary

Angels Care Home Health seeks a licensed Physical Therapist to plan, implement, and evaluate patient care plans to restore or maintain patient well-being. You will design PT treatment programs based on medical referrals and evaluations, collaborating with an interdisciplinary team.

The position offers a Monday–Friday schedule with salary plus vehicle allowance, performance bonuses, full health/dental/vision benefits, generous PTO, continuing education access, and internal promotion opportunities.

Qualifications

• Licensed Physical Therapist with current state license.
• Minimum 2 years PT experience.
• Knowledge of durable medical equipment.
