Job summary

Angels Care Home Health is seeking a Physical Therapist to plan, implement, and evaluate patient care to restore or maintain well being. The PT will organize and conduct therapy programs based on referrals and evaluations, collaborating with an interdisciplinary team to optimize outcomes.

The position requires a PT license, 2 years of experience, CPR and hepatitis profile, and knowledge of durable medical equipment. Monday–Friday schedule with benefits and advancement opportunities.

Qualifications

• Current state license as a Physical Therapist.
• Two years of experience as a Physical Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Plans, implements and evaluates patient care plans to restore or maintain patient well being.
• Plans, organizes and conducts physical therapy treatment program based on the medical referral and their evaluation.
• Collaborates with interdisciplinary team.
