Job summary

Angels Care Home Health seeks a motivated Physical Therapist to plan, implement, and evaluate patient care plans in a home health setting. You will tailor PT programs to referrals and patient progress, and work closely with an interdisciplinary team to optimize outcomes.

Requirements include an active PT license in Texas, 2+ years of experience, CPR and Hepatitis profile, and knowledge of durable medical equipment. Full-time role with benefits and growth opportunities in Centerville, TX.

Qualifications

• Active Physical Therapist license in the state.
• Minimum two years' experience as a Physical Therapist.
• Current CPR certification and Hepatitis profile.
• Knowledge of durable medical equipment (DME) required.

Responsibilities

• Plan, implement and evaluate patient care plans to restore or maintain well-being.
• Organize and conduct PT treatment programs based on referrals and evaluations.
• Collaborate with interdisciplinary team to coordinate care.
