Visiting Rehab and Nursing Services (VRNS) is seeking a dedicated Physical Therapist to provide skilled home-based care in Pittsfield, MA. This full-time role offers competitive salaried pay, a flexible schedule, and a comprehensive benefits package.

Responsibilities

Your Role as a Home Care Physical Therapist (PT):

As a Physical Therapist (PT) with VRNS, you’ll provide skilled physical therapy services directly in patients’ homes. You’ll work closely with patients, caregivers, and their care teams to improve mobility, strength, independence, and quality of life. Your responsibilities may include:

• Providing comprehensive physical therapy evaluations
• Developing individualized treatment plans based on each patient’s needs
• Providing therapeutic exercises and functional mobility training
• Assessing patients’ home environments and recommending appropriate safety modifications

• Monitoring patient progress and modifying treatment plans as needed
• Collaborating with physicians and other members of the care team
• Completing accurate and timely electronic documentation
