Job summary

Visiting Rehab and Nursing Services (VRNS) is actively seeking a compassionate Physical Therapist (PT) to join our home health team in Carver, MA. This per diem role offers up to $160 per visit, flexible scheduling, and mileage reimbursement as part of a clinician-focused organization.

You will perform skilled PT services in patients’ homes, develop personalized treatment plans, and collaborate with care teams to promote mobility and safety while maintaining thorough documentation and quality

Qualifications

• Massachusetts PT license is required.
• Minimum of one year of clinical experience.
• Afternoon availability.

Responsibilities

• Provide comprehensive physical therapy evaluations.
• Develop individualized treatment plans.
• Provide therapeutic exercises and mobility training.
• Assess home environments and recommend safety modifications.
• Educate patients and caregivers.

Your Role as a Home Care Physical Therapist (PT):

As a Physical Therapist (PT) with VRNS, you’ll provide skilled physical therapy services directly in patients’ homes. You’ll work closely with patients and their care teams to promote mobility, independence, safety, and quality of life. • Your responsibilities may include:
• Providing comprehensive physical therapy evaluations

• Monitoring patient progress and modifying treatment plans as needed
• Collaborating with physicians and other members of the care team
• Completing accurate and timely electronic documentation
