Job summary

Visiting Rehab and Nursing Services is seeking a compassionate Per Diem Physical Therapist (PT) to join our home care team in Lenox, MA. Visiting Rehab and Nursing Services (VRNS) is seeking a compassionate and dependable Physical Therapist (PT) to join our growing home healthcare team in Lenox, Massachusetts.

Your Role as a Home Care Physical Therapist (PT):

As a Physical Therapist (PT) with VRNS, you’ll provide skilled physical therapy services directly in patients’ homes. You’ll work closely with patients and their care teams to promote mobility, independence, safety, and quality of life.   • Your responsibilities may include:
  • Providing comprehensive physical therapy evaluations
  • Developing individualized treatment plans based on each patient’s needs
  • Providing therapeutic exercises and functional mobility training
  • Assessing patients’ home environments and recommending appropriate safety modifications
  • Educating patients and caregivers
  • Monitoring patient progress and modifying treatment plans as needed
  • Collaborating with physicians and other members of the care team
  • Completing accurate and timely electronic documentation

Requirements
Need to Have:
