York Nursing & Rehab Center is seeking a compassionate Physical Therapist (PT) to join our in-house rehabilitation team. We offer full-time and PRN hours, a $5,000 sign-on bonus, new graduate mentorship, and a robust benefits package to support clinical excellence and resident-centered care.

Responsibilities include evaluating residents, developing individualized plans, providing therapy per orders, educating families, documenting progress, and collaborating with the care team to maintain

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapist license or eligibility for licensure.
• New graduates encouraged to apply.
• Strong communication and clinical skills.

Responsabilités

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services in accordance with physician orders and established plans of care
• Assess mobility, balance, strength, endurance, and functional performance
• Educate residents, families, and caregivers regarding treatment goals and home exercise programs
• Monitor patient progress and adjust interventions as appropriate
• Complete accurate and timely documentation in accordance with company policies and regulatory requirements
• Communicate effectively with physicians, nursing staff, and interdisciplinary team members
• Maintain productivity standards while delivering quality patient care
