Job summary

Cliveden Nursing & Rehab Center in Philadelphia, PA is seeking a full-time Physical Therapist to join our in-house rehab team and help residents regain function. This role offers mentorship, a supportive environment, and opportunities for professional growth.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Pennsylvania (or eligibility to obtain).
• New graduates are encouraged to apply.

Responsibilities

• Evaluate residents and patients and develop individualized treatment plans.
• Provide skilled physical therapy services in accordance with physician orders and established plans of care.
• Educate patients, families, and caregivers regarding treatment goals and therapeutic interventions.
• Monitor patient progress and modify treatment plans as appropriate.
• Complete timely and accurate documentation in accordance with company policies and state and federal regulations.
• Communicate patient status and progress with physicians, nursing staff, and interdisciplinary team members.
• Assist with scheduling and accurately complete daily treatment charges.
