Job summary

Cliveden Nursing & Rehab Center in Philadelphia, PA is seeking a Full-Time Physical Therapist (PT) to join our in-house rehab team. New graduates are welcome and encouraged to apply.

You will evaluate residents, develop individualized plans of care, and provide skilled PT services in accordance with physician orders. The role emphasizes collaboration with therapy and nursing teams, patient education, and ongoing clinical education.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapist license or eligibility to obtain.
• Excellent communication and interpersonal skills.
• Passion for delivering exceptional patient care.

Responsibilities

• Evaluate residents and patients and develop individualized treatment plans.
• Provide skilled physical therapy services per physician orders and plans of care.
• Educate patients, families, and caregivers on treatment goals and interventions.
• Monitor progress and modify treatment plans accordingly.
• Complete timely and accurate documentation per company policies and regulations.
• Communicate patient status with physicians, nursing staff and the care team.
• Assist with scheduling and complete daily treatment charges accurately.
