Key Responsibilities:
- Conduct thorough assessments to evaluate patients' physical conditions and develop personalized treatment plans.
- Provide skilled physical therapy services to patients in their homes, focusing on improving mobility, strength, and function.
- Educate patients and their families on exercises and techniques to enhance recovery and prevent further injury.
- Collaborate with an interdisciplinary team to ensure comprehensive and coordinated patient care.
- Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
- Maintain accurate and timely documentation of patient care and progress.
Qualifications:
- Valid Pennsylvania Physical Therapist license.
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
- Must possess a bachelor’s or master’s degree in physical therapy approved by an accredited organization: The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, The Council on Medical Education of the American Medical Association and The American Physical Therapy Association.
- CPR certified.
- Previous experience in home health or hospice care is preferred, but not required.
- Strong communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
