Job summary

Palms Home Care is expanding its team and seeks a skilled Home Health Physical Therapist to provide individualized therapy in patients’ homes, improving mobility and independence after illness, injury, or surgery.

The role requires a licensed PT, experience in home health preferred, and ability to educate patients and families, document thoroughly, and collaborate with care teams.

Qualifications

• Current Physical Therapist license.
• Graduate of accredited PT program.
• 1–2 years of experience preferred in Home Health.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.
• Valid driver’s license and reliable transportation.
• CPR certification (current).
• Strong communication skills.
• Ability to work independently.

Responsibilities
