Job summary

Family Home Health Network in the Chicagoland area is seeking a licensed Physical Therapist to provide intermittent home health care services. You will assess patients' functional status, help develop the total plan of care, and guide families and caregivers in therapy techniques.

Responsibilities include evaluating strength, mobility, gait, instructing patients in appliance use, and prioritizing therapy needs.

Qualifications

• Baccalaureate or master’s program in physical therapy approved by an accredited organization.
• Current Illinois license as a Physical Therapist.
• Current CPR card.
• Valid IL driver’s license and auto insurance policy.
• Home Care Home Base experience preferred.

Responsibilities

• Provide physical therapy services to patients, assess therapeutic/rehabilitative/functional status, and participate in the plan of care.
• Assess muscle strength, mobility, and gait; document progress and adjust treatment as needed.
• Instruct patients, families and caregivers in the use and care of therapeutic appliances and home programs.
• Determine priority therapy needs and coordinate with the care team to achieve goals.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.
