Job summary

Family Home Health Network in the Chicagoland area is seeking a Physical Therapist to provide intermittent home health services and participate in developing the total plan of care for patients. You will assess therapeutic status and determine priority needs for therapy in collaboration with the care team.

The role includes evaluating muscle strength, mobility and gait, instructing patients and families in the use of therapeutic appliances, and ensuring safety in the home setting.

Qualifications

• Illinois license as a Physical Therapist is required.
• CPR certification is required.
• Valid Illinois driver’s license and auto insurance are required.
• Home Care Home Base experience is preferred.

Job description

Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and custodial private care services throughout the Chicagoland area.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Qualifications:

Baccalaureate or master’s program in physical therapy approved by an accredited organization

• Current Illinois license as a Physical Therapist

• Current CPR card

• Valid IL driver’s license and auto insurance policy

• Home Care Home Base experience preferred
